Forget ECG service antipsychotics
In clinical work, we often encounter long-term use of antipsychotic drugs in patients and family members ignore the return visit to the hospital regularly, advising them on a regular basis ECG, often without was prescribed a variety of reasons, resulting in serious adverse reactions, sometimes even life-threatening.
As we all know that drugs are some serious side effects, such as cardiac side effects. Whether traditional or novel antipsychotics, can lead to heart adverse reactions, ECG has been caused to predict cardiac side effects of antipsychotic drugs, an effective tool to prevent sudden death.
Antipsychotic-induced ECG abnormalities, including bradycardia, tachycardia, ECG changes, conduction block and so on. Current antipsychotic-induced QTc interval prolongation more concerned about.
QT interval refers to ventricular repolarization from the extreme end than the beginning of time, QTc interval is adjusted by the formula of QT interval. QT interval of the normal range of 320 ~ 440 ms, QTc interval prolongation may lead to syncope, ventricular fibrillation cardiac arrest and sudden death, great harm to life.
Many antipsychotic drugs can cause
QTc interval prolongation, such as chlorpromazine, clozapine, haloperidol and so on. It is reported that clozapine induced QTc interval prolongation 71.43% incidence rate, the elderly and associated with cardiovascular and cerebrovascular diseases, mental illnesses are at high risk of sudden death, the mentally ill, even taking moderate doses of antipsychotics, sudden death risk is also relatively large.
Therefore, the long-term use in patients with antipsychotics should regularly return visit to the hospital to conduct ECG monitoring, prevention of antipsychotic-induced adverse cardiac events. The current proposal is: 1. A comprehensive assessment of the baseline ECG results. 2. The treatment of periodic ECG and electrolytes. In case of tachycardia, dizziness, syncope, or epilepsy, should be immediately electrocardiogram. 3. If the ECG report tips
QT interval more than 500 milliseconds, it is recommended to immediately stop the anti-psychotic drugs.
